Dysbiotic microflora causes significant changes in the digestive and absorption function of the intestine and dramatically disrupts the normal activity of the gastrointestinal tract and the body as a whole, up to the development of sepsis (B.V. Pinegin, 1984, V.N. Krasnogolovets, 1989).
Existing methods for restoring normal flora - taking eubiotics and bacterial preparations - are not always effective. In recent years, preference has been given to the use of prebiotics, which create an optimal environment. for the development of its own microflora. One of these drugs is "Lactusan", which is a mixture of mono- and disaccharides obtained from milk sugar.
At the Essentuki resort for the treatment of dysbacteriosis, resort factors have long been used, in particular, drinking mineral waters, balneotherapy and intestinal procedures (therapeutic microclysters, siphon washing of the intestines with mineral water, mud talkers).
The purpose of our research was to study the combined effect of the natural factors of the Essentuki resort and the drug "Laktusan" in this pathology.
Observations were made on 32 patients who were treated and examined in the sanatorium named after. M.I. Kalinin and the gastroenterological department of the Essentuki clinic of the Pyatigorsk GNIIK. Of these, the majority were aged 20 to 55 years (79%) and the duration of the underlying disease (peptic ulcer, chronic acalculous cholecystitis, irritable bowel syndrome, etc.) was predominantly up to 10 years. For treatment, as a rule, patients arrived in remission or incomplete remission, the degree of concomitant dysbacteriosis varied from I to III.
In addition to a detailed clinical examination, the patients, in accordance with the tasks set, underwent a microbiological examination of feces at the beginning and end of the course of treatment.
The general microbial landscape was characterized by a decrease in the amount of obligate microflora in 23 (71.9%) patients, with a predominant decrease in bifidum and lactobacilli in 18 (56.3%) and 15 (46.9%) patients, respectively, as well as an increase in opportunistic or the presence of pathogenic microflora ( staphylococci, enterococci, hemolyzing Escherichia coli, Klebsiela, Proteus, fungi) in 18 (56%) people.
All patients were prescribed complex spa therapy, which included a motor regimen, therapeutic nutrition according to diet options N5 of the Institute of Nutrition, internal intake of moderately mineralized carbonic chloride-hydrocarbonate sodium water of Essentuki-4, warm, 3 times a day 30-40 minutes before meals at a dose of 3, 0 - 3.5 ml / kg of body weight, carbon dioxide mineral baths for 10 - 15 minutes, every other day, 8 - 10 procedures and siphon bowel lavages, 4 procedures per course of treatment. This method of treatment is traditional (basic) at the resort, it was used in 17 people in the control group of the first medical complex (LCI). In addition to the basic therapy, 15 people (LCII) took the drug "Lactusan": 1 teaspoon 2 times a day for constipation - before, with a tendency to diarrhea - after a meal.
The duration of the course of combined treatment was 10 days. According to the main clinical and paraclinical indicators, the control and main groups were comparable.
When analyzing the parameters under consideration, depending on the applied therapeutic complex, more favorable dynamics was revealed with the use of LKII compared with LKI. Thus, an increase in the number of bifidum, lactobacteria and colibacilli occurred in 10 out of 11 people (90.9%) in the experimental group versus 9 out of 17 (52.9%) in the control group (P<0.05).
Pathogenic and opportunistic microorganisms detected at the beginning of treatment by the end of the course of treatment disappeared or their titer significantly decreased in 12 (80%) patients of II LC (disappearance - 8 people, decrease - 4 people), while in patients of I LC the percentage improvement in these indicators was significantly lower - 47.1% (8 out of 17 people) and this mainly concerned a decrease in titer, and not the disappearance of opportunistic and pathogenic microflora. According to clinical studies, the advantage of LC with the use of the drug "Lactusan" over the traditional one used at the resort has also been established. Patients who took this drug in combination with resort factors noted an improvement in their general condition, a decrease in discomfort in the abdominal cavity (feelings of heaviness in the epigastrium, bloating, rumbling in the intestines), normalization of stool already on the 3rd-5th day, in contrast to patients in the control group who noted the positive dynamics of these symptoms only by the end of the second week of stay at the resort.

FINDINGS
Thus, the foregoing indicates a significant beneficial effect of the biologically active additive "Laktusan" on the state of microflora in patients with intestinal dysbacteriosis. The drug is effective not only in relation to the normalization of obligate microflora, but also in terms of the elimination of pathogenic microorganisms.
The inclusion of the drug "Laktusan" in the complex treatment of intestinal dysbacteriosis significantly improves the results of treatment of this category of patients at the resort. Probably, these components, being natural, potentiate each other's action and their total effect can and should be used in the treatment and prevention of dysbiotic disorders of the intestine.
